it's been awhile since my last post
since my graduation i tried to adjust and to start a new chapter in my life . but i had a problem in communication with the surroundings . it seems that i can't understand them and they can't understand me . am i becoming rude or they cannot appreciate what i'm saying.
it's a weird when be surrounding by a lot of people but yet you feel lonely

i hope it's just adjusting problem and it will fade away with time

i already completed two rotations out of four

i finished medicine which was very interesting and i had a good time and over all it was very high yielding

also i finished peds which was below the expectations but i had easy time back then

3 days ago my third rotation and the most stressful one started (surgery)
so far good but it's exhausting

i'm in midway through med-school finals and i'm starting thinking
am i losing my mind ? i cannot handle losing such chaotic brain
and yet i'm overwhelmed by a unique blend of emotions actually i can't describe it .it's just like experience all kind of emotions at the same moment. i reached a point where i can't tell if these days are the happiest days of my life or the other way around , but i'm sure that after 18 days i will miss these days

we are talking about the lack of basic needs of living such as food ,water shelter and health .
we are talking about 1.2 billion people in the world live in extreme poverty (less than one dollar per day).
we are not talking about luxurious live
we are not talking about fancy stuff

we are talking about the right to live....

even death can differentiate between poor and rich
according to WHO

In high-income countries more than two-thirds of all people live beyond the age of 70 and die of chronic diseases: cardiovascular disease, chronic obstructive lung disease, cancers, diabetes or dementia. Lung infection remains the only leading infectious cause of death.

In middle-income countries, nearly half of all people live to the age of 70 and chronic diseases are the major killers, just as they are in high-income countries. Unlike in high-income countries, however, HIV/AIDS, complications of pregnancy and childbirth and road traffic accidents also are leading causes of death.

In low-income countries less than a quarter of all people reach the age of 70, and nearly a third of all deaths are among children under 14. Although cardiovascular diseases together represent the leading cause of death in these countries, infectious diseases (above all HIV/AIDS, lung infections, tuberculosis, diarrhoeal diseases and malaria) together claim more lives. Complications of pregnancy and childbirth together continue to be a leading cause of death, claiming the lives of both infants and mothers.

as seen from the previous figures , death harvest the most valuable player in fighting the poverty

the human ... which make the war against poverty much harder

how we can fight poverty ?

the answer is simple

human + basic needs + free well + motivation + financial resources

yet it's unreachable nowadays because of human greed and the lack of humanity
which result in

war, poverty , pollution, death , hate , killing ,blood, greed and

politics

so to reach the answer of this conundrum we need a lot of working in sake of providing the basic needs for poor people .

and from my point of view . every body should play rule in fighting poverty even if it was a tiny rule. how?

صارلي شهر من بلش الكورس الصيفي (الحبس الاضطراري ) وللحين اعاني من اضطراب بالساعة البيلوجية(الي شكلها تفلشت ) لا نوم مثل البشر لا اكل مثل البشر لا سكن مثل البشر (الشقة ملتعن ابو خامسها) لا عطله مثل البشر بس ما اقول الا الله كريم
ودي اكتب من زمان لكن من الغلقة مالي خلق احك راسي
والي يزيد الطين بله بعدي عن (بياض الثلج) الي كانت تشاركني مصاعب الغربة و عقباتها
طبعا الدوام حدث ولا حرج من صباح الله خير تفتر ورا الدكتور ومن مريض لي مريض ومن طابق لي طابق من جناح لي جناح و اذا خلصت يحن قلب الدكتور عليك ويقولك روح اشربلك شي قبل المحاضرة الي هي عبارة عن قيلوله او حاله من التشنج العصبي اذا كان يعطيها احد عتاه قسم الجراحة
ويكتمل الفلم ويكون عندك مناوبة مسائية (يستمر دوامك الى الساعة 9 مساء وتاخذ كل الحالات الجديدة الداخلة لجسم الجراحوة عن طريق الطوارئ ) لكن الفلم ماينتهي هي
ثاني يوم الصبح راح تكون مسؤول عن التقرير الصباحي عن الحالات والذي يعني بلغة طلبه الطب استجواب لمدة ساعة بالصباح الباكر
بالله عليكم هذي حاله
ادري بتقولون اذا مو عاجبك الطب ليش داش وموهق عمرك
بقولكم انه التحلطم جزء يومي من حياة طالب الطب (لازم نتحلطم ولا مانصير دكاتره) و انه التحلطم ظاهرة صحية تقي من العديد من الامراض

Being a shadow

As usual , my vication went fast without realizing that i spent 20 days recovering from the final exams . and i'm here again starting my new rotation in surgery.
this year we had a new system where a pair of medical students joins a weekly teem in surgery and acting as shadow of the consultant following his steps , reviewing his patient's charts , doing the post -op assessment and watching the operations . it's seems great for the first time BUT when u starting the first week with the toughest surgeon in the hospital and his history in torturing the students u will not feel that good .
i'm counting the hours till the weekend hoping nothing went bad with this doctor .actually all what am thinking about it now is survival

this is the longest exam season ever i have . 11 exams with huge material , disturbed sleep ,bad mood and broken coffee maker (wen7dena 3la 3 in 1)....
till now i had 3 exams only the family medicine ,neuro and opthalmo
and my next exam will be in psychiatry (some how i feel good about it ).
so as seen,, I'm in a bad mood and shape (final sickness...) and one of the most enjoyable things that u can do while studying is listening to good music such as ..

and ..

one of my friends put a nice personal msg in msn messenger
"Criteria "A" to be a good psychiatrist is being psychotic..."

last Thursday i had the forensic exam and it was somehow good (( in the average )) after that i had a relatively good weekend and today it was along day in the radiology department where i will spend 2 weeks mastering (طل!) medical imaging .
since i started my medicals training i was interesting in medical imaging (( bringing anatomy to life ,searching of the abnormality and real time body physiology )) and as some medical students a was captivated by the beauty of the black and white films (( CT , MRI , AXR , CXR KUB , IVF ,BONE SCAN , MRA , IVU ANS U/S)) because of the story behind the picture .
dealing with people suffering makes medical student realize that there is a wide spectrum btw black and white not just in imaging but also people , treatments and outcomes .But to be save there are guidelines and protocols to follow not to treat only but to be on the safe side (( it's for ur own benefit as medz )) .why i mentioned this .
because i saw what happened in kuwait the last weeks and i realized that no one followed the (( the logic guidelines )) (( and i mean all kuwaities )) and no one want to solve the problem on the other hand everybody tries to find any tiny mistake doe by his/her enemy . unfortunately , we saw a huge wave of madness striking our society and all we did is blaming each other and no one ever tried to find the answers .

start with ur self then criticize others (( new lesson from forensics )) how said that u can not learn from dead people.

it started with beautiful dream after long day of loneness , emptiness and dullness . it was a good start for my first day in the morgue . no coffee , no water just wash ur face ( w twa'9a w 9lee) wear ur white coat and go to the morgue. the doctor has a unique sarcastic personality with good jokes about medical students and i had my share :) (( nothing personal so i'm good )) .suddenly the resident entered the office with a file for 8 months girl died 5 hrs ago in home . so we went to the identification room then to the morgue when the story started .

it was a calm room with stealy bench lay on white floor and metallic tray with needed instruments. scaple ,scissors ,forcipes meters and alot of other stuff . and she was there warped by a brown blanket covering her body and face .after preparing we entered the room . it was cold and quite and suddenly i felt of a new feeling that i couldn't define . when the doctor unwrap the girl one of my female colleagues couldn't stand the view of the baby so she left the room .

بدأ الدكتور شرحه عن كيفية اكتشاف ساعة الوفاة وعن علامات الوفاة ( اين ومتى) لكنني لم استطع ان ابعد عيناي عن ساقها الملتف و بعد التشريح ذكرني اسماعيل بالاية

the last week was great great week

- firstly aziz was discharged from hospital and he is now in home (( home sweet home ya bo 9ee5) ( he had a internal fixation by a nail ( 9ee5) so he is know officially aziz bo-9ee5 ).

and about the exam it was great... direct Qs about my case and it was easy (( surgery in my blood so it's purely genetics ;) ( according to my last name (!!فهمتها برووك ) )) .

after ortho I have forensic medicine ((( B in the morgue )) and this is the 1st day ( it was slow no cases only the seminars schedule .. pick one ... and go home ) nothing fancy like CSI but i hoping to see a good case ((evil ha)).

great weekend... great movies .

كل انسان منا يمر بمراحل في حياته تحمل الكثير من الاحداث والتجارب التي تعمل على صقل فكره لتجعل منه تحفه فريدة لذلك كل واحد منا هو باختصار تحفه فريدة وذلك بسبب فردية التجربة التي يمر بها كل انسان فينا (( للحين ما يبت شي من عندي))

Patient shocks surgeons with green blood

Surgeons operating on a 42-year-old Canadian man got a shock when they discovered dark-green blood coursing through his arteries, like Star Trek's Mr Spock.

Stunned, the medical team immediately sent his blood for analysis. The test revealed the blood discolouration was caused by sulfhaemoglobinaemia, which occurs when a sulphur atom gets incorporated into the oxygen-carrying haemoglobin protein in blood.

Doctors suspected that the patient's migraine medication caused the condition. "It is possible that our patient’s arguably excessive intake of sumatriptan, which contains a sulfonamide group, caused his sulfhaemoglobinaemia," they say.

Gradual recovery

"The patient recovered uneventfully, and stopped taking sumatriptan after discharge. When seen five weeks after his last dose, he was found to have no sulfhaemoglobin in his blood," they added.

The Canadian doctors explain that sulfhaemoglobinaemia usually goes away as red blood cells regenerate. In very extreme cases a transfusion might be necessary, they say.

Mr Spock's green Vulcan blood was supposed to have been caused by copper replacing the iron in haemoglobin.

Actinic Keratosis - A premalignant neoplasm of the epidermis caused by excessive exposure to sunlight and manifesting as an ill-marginated, erythematous, scaling, rough papule or patch.

Actinic Keratoses (AK) are located on sun-exposed areas, including the face, neck, dorsal hands and forearms, upper back and chest. AK is synonymous with solar keratosis. Occasionally, AK are more easily felt than seen, as their overlying scale is thick and firmly adherent. Lesions are occasionally tender to palpation. Fair-skinned persons, who burn easily and tan poorly, are most commonly affected. A small percentage of AK on non-mucosal skin can progress to squamous cell carcinoma . Ultraviolet light exposure induces formation of the lesions.

Microscopic examination shows a non-invasive proliferation of atypical, often crowded keratinocytes (i.e. atypical keratinocytes are confined in the epidermis and have not invaded the dermis). The atypical keratinocytes usually occupy the lower aspects of the epidermis and are confined between adnexal structures (i.e. the atypical cells often spare the epithelium of hair follicles and gland ducts). There is parakeratosis (nucleated keratin layer) overlying the atypical keratinocytes, which characteristically alternates with orthokeratosis (non-nucleated keratin layer) above the spared hair follicles. One clue to severe solar damage is elastosis in the dermis.

ومن المفرح ايضا ان ترى شخصا اخر يشاركك الذوق في الطرب والشعر ويرى بان للشعر سحر ويطرب لدى سماعه تلك الاصوات التي تعيدنا للزمن الجميل وتوقد في قلوبنا الشوق لتلك اللحظات ويؤمن بان الناس ولدوا احرارا لافرق بينهم ولا تمايز ويعلم ان مبداء ( اذا لم تكن معي فانت ضدي ) مبداء الاطفال والصغار والجهال لانه يرى بالاختلاف رحمه مادام غريمك ذو عقل رزين لانك

في الامس كنت قد انتهيت لتوي من الامتحان وكلي فرح بقدوم العطلة الاسبوعية التي تعني في عرفنا نحن الطلاب الراحة ولا شيئ سوى الراحة واذا بي اذهب الى المدينة القربية بغيت الذهاب الى السينما راجيا قضاء وقت سعيد بعيدا عن عشقي وجحيمي ( مكتبي ) واذا بي يخالجني شعور بالتوجس من قدوم خبر سيئ ينغص علي راحه بالي شعور احسست به قبل ذلك ولم يبشر بالخير
في هذا الصباح وصلني خبر بان ابن عمي او بالاحرى اخي قد اصابه مكروه
حادث سير مروع نجى منه بفضل الله ونعمته علينا لكنه يعاني الان من تبعات هذا الحادث

Suppressing Anger In Marriage Linked To Shorter Life

A new US study found that husbands and wives who suppressed their anger when one verbally attacked the other had a higher risk of early death compared to those who expressed their anger.

The study is to be published in the January 2008 issue of the Journal of Family Communication, which is not yet available online, and was the work of researchers from the University of Michigan (U-M).

The research team enlisted 192 couples living in Tecumseh, Michigan, and followed them over 17 years. The spouses were aged between 35 and 69 years when the study began in 1971.

The couples were individually asked questions about how they dealt with anger in their marriage. They were asked to imagine they were being shouted at by their partner for something that they thought was not their fault, and then say how they would respond.

Anger suppression was defined as doing at least two of the following: not show anger, not object to the attack, feel guilty after showing anger.

The couples were placed into four groups, depending on whether they communicated their anger and resolved conflicts.

Group 1 was where both spouses communicated their anger when they felt unfairly attacked by the other; groups 2 and 3 were where one spouse communicated and the other suppressed his or her anger; and group 4 was where both partners suppressed their anger and brooded.

Lead author Dr Ernest Harburg, who is professor emeritus with the U-M School of Public Health and the Psychology Department, said that:

"Comparison between couples in which both people suppress their anger, and the three other types of couples, are very intriguing."

The researchers found that early death was twice as likely to occur in the group where both partners suppressed their anger, compared to the other three groups.

As Harburg said:

"When couples get together, one of their main jobs is reconciliation about conflict."

He said that usually people are not trained in how to resolve conflict in relationships. If they had parents that were good at it, they had role models to imitate, but otherwise couples don't know how to resolve conflict.

The key, said Harburg, is how do you resolve conflict when it happens?

He explained that:

"If you bury your anger, and you brood on it and you resent the other person or the attacker, and you don't try to resolve the problem, then you're in trouble."

The study only examined anger resulting from attacks that were considered unfair or undeserved by the person on the receiving end, said Harburg. People who perceive the attack as "fair", don't get angry, he said.

Harburg and his co-researchers found that of the 192 couples they followed:

In 26 of them, both partners suppressed their anger.

There were 13 deaths over the period of study in that group.

In the remaining 166 couples, there was a total of 41 deaths (all of the three other groups combined).

In 27 per cent of the couples where both suppressed anger, one of the partners died, and in 23 per cent, both died, during the study period.

This compared to just 19 per cent of couples in the remaining three groups where one partner died and 6 per cent where both partners died.

The researchers adjusted for a range of demographic and lifestyle variables, including age, weight, smoking, blood pressure, cardiovascular risk, and bronchial and breathing problems.

The researchers said these numbers were small, but gave a preliminary view of the likely conclusions. They are in the process of gathering 30 year follow up data, which will include double the numbers of deaths.

It is also important that readers note this study only established a link between anger suppression and death. Although it ruled out certain risk factors like health, age and lifestyle, it did not establish that anger suppression caused early death. There could be an unexplored factor that linked the two.

actually this title is came from ratatouille the movie . i was thinking about my new passion ((film making )) . i suddenly realized that i have the willing to be short film maker simply because my passion of good movies . it's just amazing how the director create a world of feelings shots , faces and sounds . i knew that my passion of movies are beyond the story its about lights , music and again faces. back to ratatouille . if the rat can cook so am sure that i can make short film :)

yesterday i discovered that the sun rise at darkest hour of the day

40s women with tragic history 14 years of infertility came to the hospital to do laproscopy as an investigation to find the cause . 5 mins before my arrival to the Pt's room taking history and preparing my case for the morning rounds .the pt had million dollar news ((u r pregnant )) . when i entered the room the sadness sphere vanish i i sow the sun rising . it was marvelous

god help her in this pregnancy and give her good child .

in lectures they told as that doctors words is a weapon with 2 edges esp when u brake bad news .i didn't take this words seriously until today .

after the morning round i went to the outpatient hall to read about the afternoon seminar it was not like other days . alot of policemen, security, surgeons ,doctors, med students, yellow tape , crime seen and dead body . yes it was a suicide 19yr old boy with leukemia throw himself from the 10th floor landed in the glass garden at the entry of outpatient hall .

suddenly i realise that my words could be fetal so i will keep my mouth shut .

قبل البدء بالموضوع وجب علي توضيح بعض المصطلحات الغريبة لدى البعض
الرية : هي رحلة يقوم بها اصحاب الماشية او العاملين لديهم لجلب المياة بواسطة تنكر المياه لماشيتهم وتعتمد على عدد الرؤوس و حجم التنكر والفصل السنوي يعني بين قوسين (( اذا كنت بالصيف تحتاج رية كل خمسة ايام بالشتا راح تروي ( وهي فعل الرية ) كل عشرين يوم تقريبا ))
الحلال : المقصود بالحلال هنا الماشية من ابل واغنام
الكبرة: ............. راح اهدها لمخيلاتكم .
المتقابلات : محطات بنزين وخدمات( مركز سلطان و مطعم ابن الهيثم و محل عصير) سمية بالمتقابلات لانها تقع على الخطين بطريقة متقابلة وحدة للرايح ووحدة للجاي .

اليوم وانا بالسيارة كنت حاط على البي بي سي ( ماخذ سيارة اخوي ومافي سي دي سنع اسمعه) وكانت الحلقة عن الشأن العراقي والعنف الطائفي العنوان الاكثر شيوعا هذه الايام كنت قبل اقول احنا بالكويت بعيدين عن هذا العنف الطائفي لكن ومع الاراء الي اسمعها عن الدافع الي كان وراء استجواب وزيرة الصحة السابقة د معصومة المبارك ايقنت بانه احنا قراب وانه النفس الطائفي بداء يعصف في سماء الكويت

ليش لما تبعد عن ديرتك فترة وتردلها تحس كل شي حولك تغير وتحس انك قمت ماتعرف ناس كانوا اقرب لك من نفسك هل لانه الغربة جفى او لانه الحي يقلب .
